PURSUANT TO Arizona Revised Statutes § 33-1368, you are required to pay the total amount within FIVE (5) CALENDAR DAYS from the date of delivery of this notice, or vacate and surrender possession of the above-described premises.

If you fail to pay the full amount owed or vacate within five (5) days, your landlord will institute legal proceedings against you for recovery of the premises (Forcible Entry and Detainer), recovery of unpaid rent, court costs, and other applicable damages.

Please note: Acceptance of partial rent does not waive this notice unless expressly agreed to in writing by the landlord.
